Sports Forms Important Registration InformationSports Forms
* Yearly Physical every 13 months must be on file.
* Submit Health Assessment Form to the Nurse at nurse@srles.com If unsure of form status, please check with the school nurse
Fees are billed via FACTS and Forms must be submitted BEFORE your athlete can participate
All forms must be completed online using the Final Site Enrollment Tool and are located under your student’s upcoming school year registration.

* Sports Participation Health Record and Parent Permission
* The Concussion Informed Consent form is fillable, can be downloaded and must be uploaded to the enrollment site. *** A new form is required for each sport.***

St. Rose JV Boys Basketball Team (Grades 5-6) The St. Rose JV Boys Basketball team is made up of talented and dedicated players from grades 5 and 6.St. Rose JV Boys Basketball Team (Grades 5-6)
This team focuses on building fundamental basketball skills, teamwork, and sportsmanship while fostering a love for the game. Under the guidance of experienced coaches, the boys work hard during practices and games to develop their skills and represent St. Rose with pride.
Whether they’re learning new plays or competing on the court, these young athletes embody the spirit of determination and collaboration.

JJV Basketball (grades 3 & 4)

The St. Rose JJV Basketball program is designed for our youngest athletes in grades 3-4, with one boys team and one girls team. As a feeder program for our JV and Varsity teams, JJV focuses on teaching fundamental basketball skills, teamwork, and sportsmanship in a fun and supportive environment.
This year, we are excited to collaborate with four other local Catholic schools to offer a full basketball season, complete with games and opportunities for growth. The JJV program is a fantastic way to introduce young players to the sport and prepare them for the next level of competition while fostering a love for the game.

Track & Field St. Rose Track & Field is an exciting program for runners in grades K-8 that kicks off in March and runs through June.Track & Field
With two divisions—our younger athletes in grades K-4 and our older ones in grades 5-8—this program offers opportunities for all skill levels to develop their abilities.
Our team competes in track meets with other Catholic schools in the diocese, and we are proud to share that our K-4 division claimed the Championship Trophy in the final meet of the season!
Track & Field promotes physical fitness, discipline, and goal-setting while building confidence and teamwork. Whether sprinting, jumping, or relays, every athlete has the chance to shine.

Open to students in grades 5-8, the team focuses on developing fundamental skills, strategic gameplay, and strong sportsmanship. Whether players are learning the basics or refining advanced techniques, our dedicated coaches provide a supportive environment for growth.
Competing against other Catholic schools, the team represents St. Rose with pride and determination. Volleyball is a fantastic way for students to build confidence, foster friendships, and embrace the spirit of collaboration.

St. Rose Volleyball Season is Underway! We are excited to announce that the St. Rose Volleyball season officially begins this weekend!St. Rose Volleyball Season is Underway!
This year, we have an incredible 31 girls participating, making up two strong teams. Their first games will take place on Saturday, October 4th, and we can’t wait to see them in action on the court.
